Dr Bronze. Good morning, Mr Charles. Can you explain what happened?

Mr Charles. Well, I was standing in the kitchen helping my wife with the dishes, and I suddenly fainted – I just collapsed on the floor, so my wife called 999.

Dr Bronze. How long were you unconscious?

Mr Charles. My wife said I was out for a few minutes. She was worried sick.

Dr Bronze. I see; and did you feel strange just before it happened. Did you feel dizzy or anything?

Mr Charles. No, nothing special.

Dr Bronze. And in general, over the last few weeks, how have you been feeling?

Mr Charles. Well, now you mention it, I haven’t been feeling too well. I’ve been a bit tired, you know, feeling a bit weak.

Dr Bronze. Have you lost any weight?

Mr Charles. In fact, I have. I’ve lost a bit more than a stone in the last 6 months.

Dr Bronze. Is that because you’ve been on a diet, or been taking a little more exercise?

Mr Charles. No, neither, but as I said, I haven’ been feeling very well; I‘ve had stomach ache for the last few weeks, and then I‘ve been sick for the last couple of days – I can’t seem to keep anything down. In fact, I’ve been off work for a month.

Dr Bronze. Tell me, do you often feel thirsty.

Mr Charles. Yes, Doctor, so I’ve been drinking quite a lot of water.

Dr Bronze. How old are you Mr Charles?

Mr Charles. Sixty-four.

Dr Bronze. Could you lie down on the couch?  I’d like to examine you. You’re looking rather tanned, have you been on holiday recently?

Mr Charles. No. We went to Devon last summer, but we haven’t been anywhere since then.

Dr Bronze. How about your bowel movements, have you noticed any changes?

Mr Charles. No, nothing at all.

Dr Bronze. Well, your blood pressure’s at ninety over seventy, which is pretty low, and your heart rate’s quite high, as well as that you’ve got a slight fever. I’m just going to press gently on your tummy. Is there any tenderness?

Mr Charles. No, no pain.

Dr Bronze. Could you open your mouth, please? There are some dark patches and cracks on your tongue – seems that you’re rather dehydrated. Right, I’ll get the nurse to take a blood sample for some tests, and I’ll send you downstairs for an abdominal CT scan.

Dr Bronze. Well, Mr Charles, I’ve got the results of the complementary examinations. Your blood sugar and sodium levels are extremely low, and given the other test results, it seems you have Addison’s disease.

Mr Charles. What’s that?

Dr Bronze. It means your adrenal glands aren’t functioning properly. They’re not producing enough corticosteroids. This explains why your blood sugar is low and why you feel thirsty. We’ll have to admit you to hospital to remedy the dehydration and bring your blood sugar up to normal. Then we’ll have to initiate the cortisone therapy.  

Mr Charles. Will I be OK after that?

Dr Bronze. I’m afraid you’ll have to take corticotherapy for the rest of your life. And we’ll have to do some more tests to find out what caused the adrenal failure.
